Keeping up with the latest techniques and technologies in general and vascular surgery, the three physicians at Palm Beach Surgical Associates — 
Jack Zeltzer, M.D., FACS, vascular surgeon and incoming President of the Palm Beach County Medical Society; A. Mariano Ibarrola, M.D., FACS, general surgeon; and Arul Chidambaram, M.D., FACS, FRCS (ED), general surgeon — provide a wide range of procedures, from nonsurgical vascular interventions, such as angioplasties, to laparoscopic procedures, including colon resection and hernia repair.

A wide range of interventional services, including angiography, balloon angioplasty and atherectomy, are provided to patients at Wellington Regional Medical Center, along with the latest technology to repair abdominal aortic aneurysms in a minimally invasive fashion.

By placing a stent graft inside of an abdominal aortic aneurysm to provide reinforcement to the weakened area and prevent rupture, surgeons at Wellington Regional Medical Center offer patients a minimally invasive, endovascular approach that facilitates an earlier return to daily activities.

“By puncturing the blood vessels in the groin, we can place a stent graft inside the aneurysm to help prevent rupture,” says Dr. Zeltzer. “Before the advent of this technology, patients had to undergo a major operation that involved a large incision and a long recovery period. Using the endovascular approach, patients are often able to leave the hospital within one to two days following surgery.”

Thanks to technological advances that have occurred in recent years, many vascular procedures can now be performed in a minimally invasive fashion, with some being conducted as outpatient procedures.

“As vascular surgeons, we now have the unique advantage of offering multiple options to patients,” says Dr. Zeltzer. “Our ability to perform several different types of procedures allows us to tailor each therapy uniquely to the patient, as opposed to forcing the patient into a particular therapy because it is the only option available. Initially, we may perform a minimally invasive procedure, such as angioplasty, and move on to a traditional open procedure, such as bypass surgery, should the first option fail or if a more aggressive intervention is warranted. The development of laparoscopy has allowed physicians at Wellington Regional Medical Center to offer many general surgical procedures — such as hernia repair; colon resection; gastric resection; breast surgery; thyroid and parathyroid removal; appendectomies; and pancreatic and biliary surgery — using a minimally invasive approach.

Dr. Ibarrola has extensive expertise in laparoscopic surgery as well as in the areas of pancreatic and biliary surgery and colorectal procedures.

Caring for both benign and malignant diseases of the pancreas and biliary tree, Dr. Ibarrola offers the Whipple procedure — a surgical procedure performed to resolve cancer of the pancreas that involves removing the head of the pancreas, the duodenum and part of the biliary tree — as well as pancreatic surgery for chronic pancreatitis, laparoscopic treatment of pancreatic pseudocysts and intrabiliary bypass for benign obstructions of the biliary tree. He also performs pancreatectomies and is involved in care for colorectal cancer, surgical treatment of hemorrhoids, management of chronic rectal prolapse and transanal rectal tumor removal.

Fellowship-trained in laparoscopic surgery, Dr. Chidambaram has additional expertise in the area of dialysis access and maintenance surgery, which often involves creating an atrial venous fistula. He follows up with patients receiving hemodialysis throughout the course of their treatments, as revisions are often necessary to maintain patency and optimal performance.

Dr. Chidambaram also works closely with bariatric surgeon, Paul Wizman, M.D., FASMBS, who performs minimally invasive gastric bypass and is actively involved in the postoperative care of bariatric patients at Wellington Regional Medical Center.

“We cover the entire gamut of general and vascular surgical procedures,” says Dr. Chidambaram. “We want physicians to be aware that we remain at the forefront of technology and surgical advances. We provide the same procedures offered at academic and tertiary care centers — but close to home, in the patient’s own community. In addition, we provide follow-up calls for 
patients postoperatively.”
